Per Diem Certified Nursing Assistant (CNA)

Job Summary

Perform personal care tasks including baths, oral hygiene, dressing, and feeding patients as assigned. Monitor patient skin integrity, assist with ambulation and transfers, and administer medications within regulatory limits. Record vital signs, maintain living areas, and provide respite for families when on-site. Report significant condition changes to the Case Manager and participate in quality improvement activities. This per diem role covers patients in Salt Lake County under RN supervision. Candidates must be a certified CNA with current CPR certification, a licensed driver, and willingness to undergo background checks.

Required Qualifications

  • Must be a Certified Nursing Assistant (applicable for all States, additional requirements for California residents, see below)
  • CALIFORNIA REGULATION: Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California
  • Must have completed a minimum of 75 hours of classroom and supervised practical training
  • Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
  • Willingness to submit to a criminal background check prior to hire and annually thereafter
  • Current CPR certification
  • Ability to read and follow written instructions
  • An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
  • Ability to work autonomously with little direct supervision
  • Successfully demonstrate tact, patience and good personal hygiene
  • Ability to communicate effectively
  • Strong organizational skills
  • Understanding of plan of care documentation
  • Flexible in work hours to ensure the patients receive care as provided in the plan of care
  • Ability to empathize with the needs of the ill, injured, frail and impaired
  • Comfortable addressing issues of death/dying
  • Calm demeanor when entering the patients home

Desired Qualifications

  • Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
