Per Diem Case Management Representative

Per Diem Case Management Representative coordinates occupational health care across the continuum to promote quality and cost-effective patient care. Serves as liaison between providers, referral services, contract employers, workers’ compensation carriers, and external providers; maintains open/discharged case files; monitors referral activities; reports drug screen results; works under general supervision in a facility-based setting with varied shifts, covering for sick time and PTO. Requires effective communication, strong organizational skills, and flexibility to respond to multiple demands; typically involves coordinating plans and protocols and ensuring timely return-to-work status.
