Per diem After-Hours Nurse

Job Summary
Provide skilled nursing and hospice care in a per diem after-hours role with weekend schedules (Saturdays and Sundays, 8:30am–5:00pm) across Marin and Sonoma Counties. Responsibilities include assessing, planning, implementing, and evaluating nursing services in accordance with the plan of care; communicating with attending physicians and community resources to coordinate changes and optimize patient/family care; maintaining regular updates with Team Leaders regarding patient/family status and any issues; and delivering continuing care to patients/families in compliance with agency policies and applicable regulations. Must hold a California RN license (BSN or equivalent experience acceptable per education requirements) and may include Public Health Nursing or HPNA certifications as desirable. The role emphasizes hospice/home health care within a nonprofit hospice setting, with emphasis on quality of life for patients, families, and caregivers.
Required Qualifications
- Current license as a registered professional nurse in the State of California
- One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing
- Experience in acute care/home health and/or hospice setting preferred
- Public Health Nursing certification highly desirable
- HPNA certification preferred
