Job Summary
Conduct in-person nursing visits and assessment visits for potential hospice patients to evaluate status and provide agency service information. Use the nursing process to assess, plan, intervene, and evaluate patient needs while managing symptoms and coordinating services with the transdisciplinary team. Process admissions for eligible hospice patients in coordination with the Team Manager and escalate symptoms to providers as indicated. Schedule includes per diem coverage of two days a week or six shifts per month, with hours from 8:30am to 5:00pm. Based in Pleasanton, CA, this role supports Alameda and Contra Costa Counties, with occasional San Mateo County coverage.
Required Qualifications
- ASN or Bachelor's degree in nursing from an accredited School of Nursing
- Current license as a registered professional nurse in the State of California
- Per diem schedule
- 2 days a week or 6 shifts per month
- 8:30am - 5:00pm
- Pleasanton, CA
- Alameda & Contra Costa Counties with occasional coverage to San Mateo County as needed
- Make in-person nursing visits to patients as assigned, or as indicated when case managing
- Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals
- Provide effective management of symptoms, including prompt escalation to providers or others as indicated
- Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services
- Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status
Desired Qualifications
- Experience in acute care/ home health and/or hospice setting preferred
- Public Health Nursing certification highly desirable
- HPNA certification preferred
