Job Summary
Evaluate patients through chart review and interpret tests to inform treatment planning. Deliver comprehensive care including assessment, implementation of protocols, and patient education while managing home instructions and discharge plans. Lead interdisciplinary care conferences to review progress and delegate tasks to supportive personnel under supervision. Maintain accurate documentation of responses and revise plans based on ongoing assessments. Ensure compliance with safety standards, state regulations, and mandatory training modules. Report incidents via the Midas/RDE tool and mentor peers to enhance departmental competence.
Required Qualifications
- Bachelor's degree
- Current Physical Therapist or Occupational Therapist in the state of Arizona
- Pelvic Health Certification
- BLS (American Heart Association)
- Fingerprint Clearance Card application number
- Fingerprint Clearance Card
- Appropriate clinical competencies for the roles assigned by their department leader
- Understanding of computers
- Competence in using computers and basic software programs
