Job Summary
Plan, coordinate, and track care gaps to achieve member-centered goals while monitoring progress against recovery plans. Conduct regular contact with beneficiaries to review goals and document effectiveness, and coordinate care transitions for recent hospitalizations or life changes within 24 hours. Provide administrative and supportive supervision to program staff, including monthly reviews, performance evaluations, and individual supervision plans. Facilitate recovery groups, ensure community resource referrals, and manage 24:7 crisis response coordination as required by agency policies. Maintain medical record compliance and participate in quality improvement projects to improve service delivery.
Required Qualifications
- Meet North Carolina's definition of a Qualified Professional per 10A-NCAC 27G.0104
- A bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN)
- Two years of experience working directly with members with behavioral health conditions (if serving members with behavioral health needs) or with an I/DD or a TBI (if serving members with I/DD or TBI needs)
- Trained in quality supervisory skills through an approved CPSS supervisory training program
- Complete the CPSS Certification program and possess knowledge of the CPSS role and work, as well as understand the principles and philosophy of recovery and the code of ethics of the NC Peer Support Specialist Certification Program
- Complete mandatory CPSS supervisor training within 90 days of hire
- Maintain a valid North Carolina Driver's License
- Acceptable criminal record
- Health Registry Check that adhere to standard of employment
- Demonstrate professional work ethics
- Demonstrate knowledge of START Wellness, Inc. policies and procedures defined in the employee handbook
- Complete training requirements
- Attends all training sessions as required by the agency
- Ability to work in fast-paced environment
- Ability to maintain confidentiality
- Effectively work with all levels/division of START Wellness, Inc.
- Excellent analytical skills
- Must have proficient interpersonal skills
- Ability to make independent decisions
- Demonstrated computer skills
- Efficient with agency Electronic Health Record and/or documentation standards
- Effective oral and written communication
- Driving and travel may be required
Desired Qualifications
- If appropriately licensed, the QP may conduct the assessments
