Job Summary
Provide direct services to clients with supervision, using lived experience to model hope and resilience through coaching and role modeling. Partner with clinical teams to coordinate care, prevent crises, and help clients access benefits and community resources. Coach individuals on practical life skills, build health habits, and advocate across settings like schools and healthcare systems. Lead or support social and peer groups while maintaining accurate documentation in multiple EHR systems. This role delivers integrated behavioral health screenings and assessments within primary care clinics, supporting recovery goals for clients facing co-occurring physical and mental health needs.
Required Qualifications
- High School Diploma/GED
- Ability to read, write, and communicate effectively
- Lived or relational experience similar to the population served, including experience with mental health challenges, substance use, co-occurring conditions, or child family systems
- Actively engaged in personal recovery or resiliency plan for at least one year
- Valid Colorado driver's license with an insurable driving record
