Job Summary
Provide peer counseling and consultation to individual clients, families, and team staff while delivering case management needs that include assessing service requirements and coordinating access to necessary resources. Apply, acquire, and maintain long-term benefits for clients, offering practical help, advocacy, and direct assistance training to support daily living necessities. Deliver on-call crisis intervention and participate in daily staff organizational meetings and treatment planning reviews. Complete documentation and meet minimum service time requirements based on ACT standards. Split time 50% with ACT Team 1 and 50% with ACT Team 2. Maintain professional relationships with community figures and referral sources while adhering to Utah confidentiality laws and mandatory reporting regulations.
Required Qualifications
- Certification in Utah as a Peer Support Specialist from a curriculum approved by the Utah Department of Health and Human Services (UAPSS, USU, USARA, SBH, Optum, LBHS, DBH, FPST)
- Certified as a Case Manager in the state of Utah
- Current driver's license (if driving is a part of the job)
- Good driving record (if driving is a part of the job)
- Eligible determination from Utah-DHHS – Office of Licensing background screening
- Pass pre-employment drug screen
- Ability to lift and carry 25 lbs
- Ability to walk up and down stairs
- Ability to move around the facility, interact with clients for extended periods
- Ability to climb stairs, enter client homes, and deliver services in shelter, on the street, etc
- Willingness to accept supervision and direction
- Become CPR and First Aid Certified

- Computer literate with proficiency in Microsoft Office
- Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public
