Job Summary
Conduct general outreach, engagement, and follow-up with members while coordinating services and appointments such as wellness reminders and transportation. Engage in health promotion activities, share member circumstances with the care team, and track referrals to obtain community-based resources and social support services. Participate in case conferences and support care managers in assessing unmet health-related resource needs. This role requires flexibility to transport clients to organization-sponsored services; drivers must be 21 or older and hold a valid license with appropriate insurance. Join the Tailored Care Management team at Blue Ridge Health, a nonprofit system of Federally Qualified Health Centers serving WNC.
Required Qualifications
- High school diploma or equivalent
- Certified Peer Support Specialist
- lived behavioral health experience
- Valid Driver's License
- age 21 or greater
- certificate of insurance demonstrating at least the minimum required NC auto liability insurance
