Job Summary
Peer Support Facilitator located at the Youth Wellness Hub within Durham Community Health Centre, covering Oshawa South, Ajax Youth Wellness Hub, and other Durham Region sites. Responsibilities include facilitating peer-based support groups, providing one-on-one mentorship, connecting clients to healthcare and social services, supporting documentation and data entry, planning and delivering skills activities and drop-in programming, and contributing to program development with a focus on equity and client-centered care. Requires lived experience with mental health or substance use challenges, a 2-year diploma, and at least 2 years of relevant planning/delivery experience. Full-time temporary position with some evenings/weekends; salary range $51,000–$57,000 annually plus HOOPP. Interdisciplinary team environment and commitment to inclusive, harm-reduction approaches.
Required Qualifications
- Diploma in Social Services / Mental Health and Addictions or similar
- Lived/living experience with mental health or substance use challenges
- Minimum 2 years of program planning and delivery experience
- Ability to relate to youth in a peer capacity within an interdisciplinary health care team
- Peer support skills and confidentiality
- COVID-19 vaccination is mandatory with accommodations per policy
Desired Qualifications
- 2 year Diploma in Social Services / Mental Health and Addictions or similar
- Lived/living experience with mental health or substance use challenges, or experience as part of a marginalized population
- Minimum 2 years of program planning and delivery experience
- Demonstrated ability to relate to young people accessing services from a peer perspective and can work within an interdisciplinary health care team
- Strong self-awareness, self-care practices and can establish and maintain professional relationships with youth accessing peer services
- Peer Support Training Certificate is an asset
