Job Summary

Facilitate transitions from higher levels of care to the community by conducting needs assessments, developing daily living skills, and addressing coping mechanisms for participants. Deliver intensive emotional and practical support while assisting with housing readiness, medication management, and resolving clinical barriers to supportive housing. Provide on-call after-hours crisis intervention, accompany individuals to appointments, and navigate community services to ensure linkage with providers. Maintain collaborative relationships with referring teams and community partners to coordinate discharges and treatment plans. Document progress in Health Information Technology platforms and attend team meetings to monitor participant goals.

Required Qualifications

  • Minimum High School Diploma or equivalent or higher
  • New York State Certified Peer Specialist
  • Completion of an internship or comparable training in peer advocacy
  • Experience in providing advocacy services to people who are mentally ill and/or homeless
  • Past or current participant of behavioral health, substance services or homeless services

Desired Qualifications

  • Past or current participant of behavioral health, substance services or homeless services preferred

Additional Requirements

  • Job requires on-call coverage and willingness to work flexible hours
