Job Summary

Provide outreach to clients and families via phone and in person, and facilitate engagement in services through linkage to community resources. Participate in crisis assessments within the multidisciplinary team, offering warm handoffs to all levels of treatment while utilizing motivational interviewing to support recovery readiness. Coordinate care planning with providers to ensure goal-directed transitions and act as a resource for community-based recovery supports. Deliver opioid overdose education and train individuals on Narcan use, while maintaining thorough documentation in the Electronic Health Record. Complete required training in de-escalation and crisis intervention techniques, and report to work during inclement weather or emergencies.

Required Qualifications

  • High school diploma or equivalent
  • NYCPS (New York Certified Peer Specialist) or CRPS (Certified Recovery Peer Advocate) certification
  • Relevant life experience
  • Excellent interpersonal and verbal and written communication skills
  • Ability to work both independently as well as part of a team
  • Responsiveness to deadlines
  • Valid and clean New York State Driver's License
  • Ability to operate a vehicle in a safe manner under all driving conditions
  • Evening and Weekend hours

Desired Qualifications

  • Experience as a peer, paid or volunteer
  • Proficient computer skills and typing
  • Experience with E.H.R.
  • Microsoft office
