Job Summary

Conduct outreach and case management for frequent users of emergency services, criminal justice, and county resources, including encampments, shelters, and treatment facilities. Engage participants with serious mental health, substance use, or chronic health conditions to re-establish contact, complete initial intakes, and maintain BIRP case notes. Link clients to community services for housing, benefits, and recovery planning while advocating within the court system. Assist with Wellness Recovery Action Plans and navigate public benefits like General Assistance and SSI. Enter accurate data for program evaluation and Medi-Cal billing. Requires personal experience with recovery, six months of field work, and a valid California driver's license.

Required Qualifications

  • High school diploma or equivalent
  • Personal life experiences with recovery from addiction, homelessness, and/or mental health recovery
  • Six months of field experience working with people experiencing homelessness
  • Use of personal vehicle
  • Proof of a valid and current California Driver's License
  • Current insurance
  • Clean DMV record
