Job Summary
Provide personalized supports to criminal justice involved clients navigating behavioral health and reentry services, utilizing coaching, mentoring, and role modeling based on lived experience. Facilitate bimonthly or monthly groups for skill-building and resource linkage, while assessing social determinants of health to coordinate care for primary care, medication-assisted treatment, and employment. Connect clients with essential community resources and foster engagement through one-on-one and group settings. Work primarily in SAGE offices across Maricopa County with a caseload of approximately 50 clients, requiring a valid Arizona driver's license, reliable transportation, and a clean driving record.
Required Qualifications
- Peer Recovery Support Specialist (PRSS) credential via an AHCCCS-recognized credentialing agency
- Forensic PRSS credential
- Minimum 1 year in self-managed recovery
- 6 months post-discharge from all SAGE programming
- Valid Arizona Fingerprint Clearance Card
- Valid Arizona Level One Fingerprint Clearance Card
- Valid Arizona driver's license
- Reliable transportation
- Current auto insurance
- Clean driving record
- 21 years of age or older
- Ability to work cooperatively with people of diverse backgrounds
- Good verbal and interpersonal skills
