Job Summary
Assess clients' personal, medical, emotional, social, and environmental situations through home, community, and office visits to plan treatment courses and coordinate individual recovery plans. Provide client-centered, strengths-based, and trauma-informed services including crisis/on-call coverage, medication training, and advocacy for entitlements like SSI and Medicaid. Model recovery-oriented living skills, assist with independent living tasks, and document progress in real time within the Electronic Medical Record. This role supports clients with severe mental illnesses and co-occurring substance use disorders at CST Onyx's Chicago office. Successful candidates must acquire a CRSS credential within 12 months and participate in daily team meetings to collaborate on shared caseloads.
Required Qualifications
- High School Diploma or GED and two years of supervised clinical experience in a mental health setting
- Associate's or Bachelor's degree in any field
- Must be a past or present consumer of mental health services and willing to share their personal story of recovery with our clients
- Valid Illinois driver's license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
- Acquire a CRSS credential within 12 months of hire
- IM+CANS certification preferred and may be required to obtain IM+CANS certification within 30 days of hire
Desired Qualifications
- Experience working with people with mental health and/or substance abuse diagnoses preferred
- Experience in community-based services preferred
