Job Summary
Assess clients' personal, medical, emotional, social, and environmental situations through home, community, and office visits to plan treatment courses. Provide individually based motivational treatment and support to assist clients in recovery from mental illness and co-occurring substance use disorders. Coordinate individual recovery plans with clients, support systems, and care providers while modeling good communication and recovery-oriented living skills. Complete case management tasks, daily progress notes, and documentation in the Electronic Medical Record including IMCANS and LOCUS. Assist clients with independent living skills, entitlement applications, and medication training, accompanying them to appointments and managing representative payeeship services. Provide crisis and on-call coverage as directed and actively participate in Peer Support meetings and daily team communications.
Required Qualifications
- High School Diploma or GED and two years of supervised clinical experience in a mental health setting
- Associate's or Bachelor's degree in any field
- Must be a past or present consumer of mental health services and willing to share their personal story of recovery with our clients
- Valid Illinois driver's license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
- Acquire a CRSS credential within 12 months of hire
- IM+CANS certification preferred and may be required to obtain IM+CANS certification within 30 days of hire
Desired Qualifications
- Experience working with people with mental health and/or substance abuse diagnoses preferred
- Experience in community-based services preferred
