Peer Recovery Coach

Job Summary
Provide peer recovery services to individuals identified as having risk for substance misuse or substance use disorder, focusing on connecting individuals to community resources and ensuring smooth care transitions to treatment services. Utilize Motivational Interviewing and trauma-informed approaches to build rapport, promote wellness, and support recovery through individual and group sessions in community and clinic settings. Conduct recovery groups, educate patients to develop self-management skills, assist with setting recovery goals, and coordinate with treatment providers; maintain confidentiality and documentation in the EHR, and monitor treatment outcomes to improve quality of care. Based in Trinity Health’s Livonia, MI system, with emphasis on rehabilitation, care transitions, and collaborative support to enhance patient participation and recovery outcomes.
Required Qualifications
- High School Diploma or equivalent
- Lived experience with at least 2 years in recovery
- Experience in working with behavioral health conditions
Desired Qualifications
- Lived experience with at least 2 years in recovery
- High School Diploma or equivalent
- Peer Recovery Coach Certification preferred
- Experience in working with patients who have behavioral health conditions
- Knowledge of care transitions/Zero Suicide model preferred
