Peer Navigator (Case Manager)

Job Summary
Supportive Peer Navigator and Case Manager responsibilities include overseeing all aspects of care for assigned members, coaching and mentoring based on the peer recovery process, assessing mental and physical health and psychosocial needs, collaborating with health homes and community resources to develop and implement an Individualized Service Plan (ISP), ensuring services are delivered to meet outcomes, coordinating therapy, employment, and Substance Use Disorder services, facilitating communication among health care team members, promoting reintegration into the community, and maintaining timely documentation in behavioral health records. Requires strong leadership, organizational skills, and the ability to coordinate with multiple stakeholders; must have a valid Arizona driver’s license with a clean Motor Vehicle Record, be able to provide transportation as needed, and complete required certifications and TB testing as applicable.
Required Qualifications
- High School/GED + 2 years’ behavioral health experience
- Bachelor’s degree or Master’s degree in a behavioral health field
- Peer - Individuals must have first-hand knowledge of a behavioral health condition or substance abuse issue, whether or not they received treatment or medication or identify as a family member of someone in recovery.
- Attending Peer Support Certification training within the first 3 months of employment
