Job Summary
Peer Mentor responsible for delivering peer support services to clients in recovery, establishing supportive relationships, facilitating individualized recovery goals, crisis intervention, care coordination, and navigation of behavioral health and societal resources. Roles include crisis response, documentation in electronic health records, collaboration with medical and behavioral health teams, transporting clients when needed, and applying trauma-informed, harm-reduction, and recovery-oriented practices to promote stability and self-determination. Must have lived recovery experience and pursue CRM credential; position is full-time with two shift options.
Required Qualifications
- High school diploma or GED
- lived experience with substance use recovery
- CRM credential or ability to obtain within six months
- eligibility for Medicaid provider in Oregon
- criminal history background check clearance
- driver's license and transportation if driving is required
- ability to travel ~36% or more of work time
