Job Summary
Engage consumers in health promotion planning, motivational interventions, and lifestyle changes to support chronic condition management. Develop self-management goals with consumers and families, monitor progress, and provide skill development for nutrition, exercise, sleep, and substance avoidance. Facilitate community support, wellness groups, and transportation to appointments while coordinating care with RN/LPN staff and medical providers. Orient consumers to primary care principles and conduct wellness assessments. Participate in team meetings and manage data entry with strict compliance to reporting standards.
Required Qualifications
- Bachelor's degree
- Possess strong customer service and listening strengths
- Knowledge of mental illness and ability to act as a good coach for this population
- Possess ability to function in a team environment
- Possess excellent written and oral communication strengths and problem solving abilities
- Possess valid driver's license
- Strong data entry skills, in multiple electronic platforms
- Effective Time management and understanding of deadlines
- Understanding compliance in documentation and reporting standards within deadlines
