Job Summary
Assess clients' personal, medical, emotional, social, and environmental situations through home, community, and office visits to plan treatment courses. Implement assertive engagement strategies to bring potential clients into services and link them to recovery groups, housing, and benefits. Provide client-centered, strengths-based support utilizing motivational interviewing and lived experience to assist individuals experiencing homelessness with substance use and mental health recovery. Conduct outreach visits to encampments and shelters, accompany clients to appointments, and help them navigate entitlements and housing waitlists. Complete daily progress notes per Trilogy standards, participate in Peer Support meetings, and collaborate within a multi-disciplinary team model. Acquire CRSS credential within 18 months and share personal recovery stories to build trust.
Required Qualifications
- 21 years of age or older
- GED or high school diploma
- Skill in the delivery of rehabilitative services to adults
- The ability to work within a provider agency's structure and accept supervision
- The ability to work constructively with individuals receiving services, other providers of service, and the community
- CRSS certification through IAODAPCA within 12 months of hire
- Must be a past or present consumer of mental health or mental health and substance abuse services
- Willingness to share their personal story of recovery with our clients
- Valid Illinois driver's license
- Daily access to a well-maintained vehicle
- $100,000/$300,000 liability insurance
Desired Qualifications
- Experience working with people with mental health and/or substance abuse diagnoses
- Experience in community-based services
