Peer Case Manager- CTI Program

Job Summary
Develop person-centered care plans and provide crisis management for up to 15 individuals with serious mental illness, substance use, or trauma. Link clients to community resources, engage them in skill building, and coordinate care across providers while maintaining documentation in the electronic health record. Attend weekly case presentations, collaborate on multi-disciplinary teams, and troubleshoot treatment challenges through case conferences. Adhere to trauma-informed care principles and participate in the CTI Learning Collaborative and Learning Community. Provide core services including psychosocial rehabilitation and health services coordination. Maintain client confidentiality and complete all required agency and state trainings.
Required Qualifications
- At least 18 years of age
- High-school diploma or equivalent
- At least six (6) months of direct care experience with individuals with serious mental illness
- Certified Peer Specialist through nypscb.or
- Willingness to obtain certification within six (6) months of hire
- Valid driver's license
- Acceptable driving record
- Must be part of the after-hours on-call rotation
