Peer Bridger/Mentor-(OMH) SS Queens Adult Homes

Conduct outreach, engagement, and acculturation for participants struggling with mental illness, homelessness, and substance abuse. Provide crisis intervention services and supportive counseling to reduce self-destructive behaviors while assisting clients in teaching daily living skills. Build participant esteem by engaging them in mutual interest activities, accompanying them to apartment viewings, and aiding in obtaining entitlements. Encourage access to specialized and natural community support through groups promoting socialization and recovery. Collaborate with case management staff on client needs and document all interactions. This part-time role (15–20 hours/week) supports recovery, re-housing, and coping skills within the OMH Adult Homes Scatter Site Program.
