Pediatric Transitional Nurse – Registered Nurse (RN) Licensed Practical Nurse (LPN)

Provide skilled pediatric nursing 1:1 care in patients' homes, administering medications, treatments, and therapies while performing assessments and ongoing evaluations. Support hospital-to-home transitions for high-acuity patients, including trach and vent care, and educate families during discharge support and care coordination. Offer first-shift and interim coverage as needed, collaborating with Clinical Care Managers and responding appropriately in emergency situations. Train and support permanent field nurses during transitions. Full-time position with guaranteed hours, day and night shifts available, plus on-call and staffing coverage responsibilities.
