Job Summary
Provide skilled pediatric nursing 1:1 care in patients' homes, administering medications, treatments, and therapies while performing patient assessments. Support hospital-to-home transitions for high-acuity patients, ensuring safe discharges and preventing readmissions through interim coverage and care coordination. Educate families and caregivers, respond to emergency situations, and train permanent field nurses during transitions. Requires Trach and ventilator management experience with high-acuity pediatric patients. Full-time position with day and night shifts, on-call support, and local travel.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (PA)
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- Pediatric home health experience
- Experience with Trach care and Ventilator management
- High-acuity pediatric patients
- Full-time position
- Day and Night shifts available
- On call and Staffing coverage responsibilities as needed
- Field-based role with local travel
