Job Summary
Provide skilled pediatric nursing 1:1 care in patients' homes, performing assessments, administering medications, and supporting hospital-to-home transitions for high-acuity patients. Educate families and caregivers while assisting with discharge support and care coordination. Respond appropriately in emergency situations and provide first-shift or interim coverage as needed. Train and support permanent field nurses during transitions. Collaborate with Clinical Care Managers and interdisciplinary teams. Requires active RN or LPN license, trach and ventilator experience, and pediatric home health background. Full-time role with day and night shifts available.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (PA)
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- Pediatric home health experience
- Experience with Trach care and Ventilator management
- High-acuity pediatric patients
