Job Summary
Provide skilled pediatric nursing 1:1 care in patients' homes, performing assessments, administering medications, and managing trach, vent, and G-tube needs. Support hospital-to-home transitions for high-acuity patients by ensuring safe discharges, preventing readmissions, and providing interim coverage until permanent staffing is established. Educate families, coordinate care with interdisciplinary teams, and respond to emergency situations. Full-time position with guaranteed hours, day and night shifts available, and on-call responsibilities.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (PA)
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- Pediatric home health experience
- Experience with Trach care and Ventilator management
- High-acuity pediatric patients
- Full-time position
- Day and Night shifts available
- On call and Staffing coverage responsibilities as needed
- Field-based role with local travel
