Job Summary
Provide skilled pediatric nursing 1:1 care in patients' homes, including administering medications, treatments, and therapies. Support hospital-to-home transitions for high-acuity patients by performing assessments, educating families, and coordinating discharges to prevent readmissions. Offer first-shift and interim coverage as needed while collaborating with Clinical Care Managers and responding to emergency situations. Train and support permanent field nurses during transitions. Requires Trach and vent experience with full-time hours, day and night shifts, and local travel.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (PA)
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- Pediatric home health experience
- Experience with Trach care and Ventilator management
- High-acuity pediatric patients
