Job Summary
Provide skilled pediatric nursing 1:1 care in patients' homes, administering medications, treatments, and therapies while performing ongoing assessments. Support hospital-to-home transitions for high-acuity patients, including trach and ventilator management, and educate families during discharge. Offer first-shift and interim coverage as needed, collaborating with clinical teams and training permanent field nurses. Ensure safe, successful discharges to prevent readmissions and respond appropriately to emergency situations. Full-time position with guaranteed hours, day and night shifts available, plus on-call responsibilities.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (PA)
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- Pediatric home health experience
- Experience with Trach care and Ventilator management
- High-acuity pediatric patients
