Job Summary
Oversee the Pediatric team and plan, organize and direct hospice care, with emphasis on nursing and community health education/experience. Key duties include enhancing communication for field teams, conducting weekly IDT meetings, guiding new employees with orientation and education in hospice care, performing quarterly performance check-ins, ride-along visits, maintaining a small caseload, and completing initial and ongoing patient/family assessments to determine hospice needs. Requires RN with hospice experience and supervisory background, plus a valid driver’s license and insured vehicle.
Required Qualifications
- RN with minimum of 3 years of Hospice experience
- Prior supervisory experience
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order.
