Job Summary
Develop and implement individualized treatment plans for children with medically complex needs, providing direct physical therapy services in the home setting. Support gross motor development, mobility, strength, coordination, and functional independence while monitoring progress and adjusting plans to achieve meaningful outcomes. Recommend adaptive equipment, train families on proper use, and partner closely with parents to ensure carryover between visits. Collaborate with an interdisciplinary team to deliver coordinated, family-centered care while maintaining accurate documentation of evaluations, daily notes, and progress reports.
Required Qualifications
- Graduate of an accredited Physical Therapy program, DPT or equivalent
- Active Physical Therapy license in Texas
Desired Qualifications
- One year of experience preferred
- Pediatric experience preferred, but not required
- Home health experience is a plus
- Strong communication skills and the ability to build trust with children and families
- A compassionate, dependable, and detail-oriented approach to care
- Commitment to providing high-quality, family-centered therapy
