Job Summary
Provide direct patient care to infants, children, and adolescents in their homes according to the active plan of care. Perform comprehensive patient assessments, administer prescribed medications and treatments, and document clear clinical notes in the KanTime EMR. Notify the RN Case Manager of condition changes and educate families on diagnosis, treatment, and prevention. Collaborate with therapists and medical professionals to ensure quality care while adhering to infection control procedures and state regulations. Full-time, part-time, and PRN positions available with competitive pay, sign-on bonuses, and tuition reimbursement.
Required Qualifications
- Current State Licensed Vocational Nursing (LVN) License
- Current CPR Certification
- Must have reliable transportation to office and patients' homes
- Must be able to bend, reach, stand, stoop, sit, twist, talk, and hear
- Must be able to push and pull 50 pounds
- Must be able to support 50 pounds of weight
- Must be able to lift 50 pounds
- Must be able to assist with weights of more than 100 pounds
- Must be able to carry equipment and supplies
- Must be able to see information up to 24 inches away
Desired Qualifications
- One (1) Year Home Health Experience Preferred
- One (1) Year Pediatric Experience Preferred
