Job Summary
Role focuses on educating healthcare providers and families about pediatric hospice and palliative care, serving as the primary relationship-builder and referral coordinator between pediatric hospice services and hospitals, physicians, clinics, families, and community organizations. Responsibilities include promoting admissions, cultivating referrals, and supporting continuity of care for children with life-limiting illnesses; requires travel to patient homes and facilities, and active engagement with interdisciplinary teams to develop and monitor spiritual and care plans.
Required Qualifications
- Master’s degree in Divinity, Theology, Christian counseling or related degree for hires after 5/26/2024 OR Bachelor’s degree in Divinity or related degree for hires prior to 5/26/2024
- Ordination or commissioning/endorsement from recognized ordaining body
- One unit of Clinical Pastoral Education (CPE) or in process within six months
- EMR/EHR experience preferred; computer skills required
- Ability to travel and have reliable transportation
- Valid driver's license and automobile insurance per policy
- Must be able to physically access homes, hospitals, nursing homes, and ALFs
- Must communicate clearly in English
