Job Summary
Develop and follow individualized nursing care plans incorporating the client's specific needs, including pediatric clients with G-tube buttons. Perform comprehensive physical, psychosocial, and environmental assessments, completing OASIS data sets as appropriate. Provide skilled interventions to achieve realistic client outcomes and report significant status changes to the physician and team. Maintain systematic client records and participate in team meetings to clarify reporting responsibilities. Requires a valid Indiana RN license, driver's license, and background check; Trach or vent experience preferred. Full-time day shift in LaPorte with flexible scheduling, no required 12-hour shifts, weekends, or holidays.
Required Qualifications

- Willing to Travel to Nearby Clients in Need of Assistance
