Pediatric Homecare Nurse | RN or LPN

Conduct comprehensive assessments of client status, including physical, psychosocial, and environmental parameters, while completing OASIS data sets as appropriate. Develop and follow individualized Nursing Care Plans that incorporate specific client needs, reporting significant status changes to physicians and the team in a timely manner. Provide skilled interventions to achieve realistic outcomes and maintain systematic records documenting assessment, planning, and evaluation. Coordinate client care and participate in other duties as assigned. Full-time or part-time roles available with 7 nights a week (11pm-7:30am) scheduling.
