Job Summary
Provide direct patient care to infants, children, and adolescents in their homes according to active plans of care. Perform comprehensive assessments, administer prescribed medications and therapies, and document timely clinical notes. Educate patients and families on diagnosis, treatment, and prevention while collaborating with professional disciplines. Notify the RN Case Manager of condition changes and adhere to infection control procedures and state regulations. Requires current LVN license, CPR certification, and reliable transportation. Flexible scheduling available with sign-on bonus opportunities.
Required Qualifications
- Current State Licensed Vocational Nursing (LVN) License
- Current CPR Certification
- Must have reliable transportation to office and patients' homes
- Must be able to bend, reach, stand, stoop, sit, twist, talk, and hear
- Must be able to push and pull 50 pounds
- Must be able to support 50 pounds of weight
- Must be able to lift 50 pounds
- Must be able to assist with weights of more than 100 pounds
- Must be able to carry equipment and supplies
- Must be able to see information up to 24 inches away
Desired Qualifications
- One (1) Year Home Health Experience Preferred
- One (1) Year Pediatric Experience Preferred
