Job Summary
Pediatric Coordinator role focusing on bilingual pediatric care coordination within home healthcare. Responsibilities include managing a pediatric caseload with supervision visits, care plan updates, service plan meetings, ensuring documentation standards and timelines, coordinating with Medical Records, PAR, HR, Intake, Nursing, and Quality to resolve issues, maintaining accurate client records, promoting family-centered communication, and implementing workflow/process improvements within the Pediatric Department. Requires strong organizational, communication, and regulatory interpretation skills; proficient with Outlook and EHR; capable of handling escalations and collaborating across interdisciplinary teams. Ideal for candidates who are bilingual in English and Spanish, with experience in Medicaid/pediatric HCBS contexts and a commitment to compliant, high-quality service delivery for children and families.
Required Qualifications
- Minimum 3 years working with children and adolescents with disabilities, including care coordination or case management.
- Familiarity with HCBS waivers, pediatric IHSS, and Personal Care programs.
