Pediatric Clinical Pharmacist - Ambulatory Clinics

Job Summary

Interpret and evaluate provider orders for accuracy, perform medication reconciliation, and identify therapeutic problems such as drug interactions or improper dosing. Monitor medication therapy for response and adverse reactions, collaborate with providers under a Collaborative Drug Therapy Management agreement to initiate or modify treatment, and counsel patients on medication use, side effects, and storage. Conduct prospective and retrospective drug utilization reviews, facilitate prior authorizations, and coordinate care transitions between inpatient and outpatient settings. Precept students, interns, and residents while maintaining appropriate records and enforcing practice guidelines. Manage competing demands and frequent interruptions in an ambulatory clinic setting requiring independent decision-making and sound professional judgment.

Required Qualifications

  • A current Pharmacist's license issued by the Oregon State Board of Pharmacy
  • Licensure in the State of Washington within 6 months
  • Pharm.D. or BS in Pharmacy with equivalent experience
  • Pediatric and/or ambulatory pharmacy practice experience
  • Advanced Pharmacy Practice Residency OR 5 years of equivalent experience
  • Experience in clinical program development
  • Direct patient care or medication therapy management experience
  • State of Oregon Preceptor License Additional Certification (BCPS, BCACP, BCPPS, etc.)
