Job Summary
Conduct comprehensive diabetes self-management education and training for patients, families, caregivers, and staff at the Pediatric Diabetes Center and affiliated hospitals. Collect blood glucose data, monitor outcome measurements, and maintain evidence-based protocols for hypoglycemia and hyperglycemia management. Support physicians in clinical co-management by identifying barriers to care, coordinating transitions, and modifying medical regimes based on critical assessments. Develop diabetes education curricula, establish policies and standards of practice, and encourage research to validate and improve Diabetes Self-Management and Training. Participate in quality initiatives and performance development using evidence-based practices to achieve identified treatment goals.
Required Qualifications
- Certified Diabetes Educator (CDE)
- Registered or licensed by an appropriate accrediting body in his/her respective field
- Minimum of a bachelors degree
- Certified by the National Certification Board of Diabetes Educators
- Basic Life Support for Healthcare Providers
- Background in pediatrics
- Minimum of one year of diabetes education either in hospital setting or outpatient practice
- Clinical competence in the use of diabetes medications including insulin
- Clinical management of all patients with diabetes
- All aspects related to education and successful use of continuous subcutaneous insulin pumps
- All aspects related to education and successful use of continuous glucose monitoring
- Management and modification of individual diabetes plan of care based on optimizing glycemic control and specified targets
- Diabetes Care & Education license
- Diabetes Care_Educ specialist license
- Ability to effectively communicate with all levels of the organization
