Job Summary
Perform patient-centered, evidence-based nursing care activities under LPN or RN supervision, assisting with personal hygiene, mobility, activities of daily living, vital signs, and medication administration. Collaborate with the care team to ensure quality, cost-effective, safe patient care while identifying ways to streamline processes and lower costs. Respectfully address patient concerns, escalate risks to prevent crises, and reinforce patient education to improve outcomes. Maintain therapeutic relationships with patients, families, and providers while documenting all care tasks.
Required Qualifications
- Must be 18 years of age, except for Peabody Manor, The Heritage or Juliette Manor
- Valid State of Wisconsin CNA Certification
- American Heart Association Healthcare Provider Basic Life Support (BLS) certification required
- Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance
- Manual dexterity and hand-eye coordination to perform patient care procedures
- Position requires compliance with department specific competencies
Desired Qualifications
- High school diploma or GED preferred
