Job Summary
PDPM/MDS Coordinator in a Skilled Nursing Facility oversees the MDS process and PDPM-related documentation to optimize reimbursement and resident outcomes. Responsibilities include leading MDS coordination (initial, significant change, quarterly, and annual assessments) with the interdisciplinary team; ensuring compliance with state and federal regulations; accurate coding and documentation to support PDPM reimbursement; developing individualized care plans based on MDS findings; conducting audits and quality checks; collaborating across nursing, therapy, social work, and other healthcare professionals; identifying opportunities to improve reimbursement and clinical processes; providing staff education on MDS requirements, coding guidelines, and PDPM practices; ensuring ongoing regulatory compliance and data reporting for management decisions; maintaining awareness of changes in regulations and guidelines and implementing updates in MDS processes.
Required Qualifications
- RN or LPN licensure preferred
- in-depth knowledge of MDS assessment process and PDPM guidelines
- experience with MDS coordination
- familiarity with MDS software systems
- ability to collaborate with interdisciplinary team
- strong analytical and attention-to-detail skills
- proficiency with computer software and reporting tools
- knowledge of quality assurance and performance improvement principles
- experience with EHR systems and healthcare technology
- ability to adapt to regulatory changes related to MDS and reimbursement
