Job Summary
Night-shift PDN RN/LPN in home health for Lexington, OH; provides skilled nursing visits, follows care plans, monitors medications, administers injections/IV care, and venipuncture; collaborates with RN Case Manager; documents in clinical records; travels within Richland County to cover assigned region; experience with case management, wound care, PICC/PORT/IV care, lab draws, and familiarity with OASIS/Non-OASIS assessments is highly desirable; CPR and First Aid certificates are highly desirable; strong interpersonal and documentation skills required; travel flexibility and ability to support a 9 PM–5 AM schedule with 56 hours per week.
Required Qualifications
- Active LPN or RN license in the state of Ohio
- Graduate of an accredited school of professional nursing
- 1 year in home health industry + 1 year of nursing experience
- Additional Home Health Nursing experience/familiarity highly desired
- Active CPR and First Aid certificates (recertification can be provided)
- Willingness to pass state and federal background check
- Ability to meet basic job requirements
- Current driver’s license or state ID and reliable transportation
