Job Summary
Night shift PDN role delivering skilled nursing care in home health for Lexington, OH (Richland County) and surrounding areas. LPN or RN with current Ohio license required; 1+ year home health experience preferred, plus case management, wound care, PICC/PORT/IV care, lab draws, and OASIS/Non-OASIS assessments are desirable. Role is long-term with potential to be FT (56 hrs/wk) or PT, with travel within the primary service area. Compensation ranges posted: LPN $28-30/hr and RN $35-40/hr, with benefits for full-time staff and travel reimbursements. Night hours are 9 PM–5 AM, seven days per week; some cases may be split between two PDNs. Mileage reimbursement provided; background checks and driver requirements apply. Reports to RN Case Manager/Clinical Administrator; essential nursing functions include direct patient care, care-plan adherence, pharmacologic monitoring, documentation, and coordination with RNs and physicians. This position emphasizes autonomy, client-centered care, and growth opportunities within the agency.
Required Qualifications
- Active LPN or RN license in the state of Ohio
- Graduate of an accredited school of professional nursing
- Minimum 1 year in home health or similar nursing experience
- One year direct patient experience with vent care (preferred)
- Ability to pass background check
- Reliable transportation and valid driver's license
- Willingness to travel within assigned region
- CPR and First Aid certificates desirable
