Job Summary

Provide skilled nursing care and coordinate patient care plans under RN supervision for homebound individuals, adhering to physician orders and confidentiality standards. Continuously assess patient conditions, document all nursing actions and responses in electronic systems, and implement infection control measures per OSHA standards. Recognize changes in patient needs requiring intervention to prevent or reduce risk, while educating families on disease processes and self-care techniques. Maintain professional accountability by complying with state regulations and the Nurse Practice Act, and participate in ongoing education to sustain clinical competencies. Requires an active LPN/LVN license, current CPR certification, and reliable transportation; weekend shifts available in San Antonio, TX.

Required Qualifications

  • Active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
  • Compact licenses must be transferred to your state of residence within 90 days
  • Current CPR certification (with hands-on component)
  • TB skin test (current within last 12 months)
  • Reliable transportation
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
  • One (1) year of pediatrics experience (South Carolina)
  • One (1) year of experience required working under current nursing license (California)
  • One (1) year of experience required working as a licensed nurse (Louisiana)
  • Continuing Education as required by state
  • Six months prior hands-on nursing experience preferred but not required
  • Must be able to speak, write, read and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases

Desired Qualifications

  • Six months prior hands-on nursing experience preferred but not required
  • Home health experience
  • Attention to detail

  • Effective problem-solving and conflict resolution
  • Good organization and communication skills
  • Must have visual and hearing acuity
  • Must have strong sense of smell and touch
  • Must be able to sit and climb stairs
  • Must have the ability to prolong walking, standing, bending, kneeling, reaching, twisting
