Job Summary

Deliver and coordinate quality patient care in compliance with physician orders while continuously assessing patient conditions and reporting changes to supervisors. Document all activities, assessments, nursing actions, and care coordination in a timely manner using electronic tools, and participate in updating nursing care plans. Implement appropriate nursing interventions based on assessments to achieve expected outcomes, recognize changes in patient needs, and educate patients and families regarding disease processes and self-care techniques. Maintain infection control measures and comply with state regulations, the Nurse Practice Act, and OSHA standards while functioning in home environments.

Required Qualifications

Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
Compact licenses must be transferred to your state of residence within 90 days
Current CPR certification (with hands-on component)
TB skin test (current within last 12 months)
Must have reliable transportation
Must be able to speak, write, read and understand English
