Job Summary

Provide skilled nursing care and coordinate patient outcomes under RN supervision while adhering to physician orders and confidentiality standards. Continuously assess patient conditions, document all activities and nursing actions in electronic systems, and update care plans to achieve expected outcomes. Recognize changes in patient needs requiring intervention to prevent or reduce risk, and educate patients and families on disease processes and self-care techniques. Maintain infection control measures per OSHA standards and participate in continuing education to sustain competency. Ensure compliance with state regulations, the Nurse Practice Act, and Aveanna policies while functioning in diverse home environments.

Required Qualifications

Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
Compact licenses must be transferred to your state of residence within 90 days
Current CPR certification (with hands-on component)
TB skin test (current within last 12 months)
Must have reliable transportation
Must be able to speak, write, read and understand English
Must be able to travel
Must be able to lift 50 pounds
Must be able to sufficiently reposition patients and move equipment without assistance
Must be able to sit and climb stairs
Must have visual and hearing acuity
Must have strong sense of smell and touch
Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
Possible exposure to blood, bodily fluids and infectious diseases
Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
Must comply with state and federal regulations, accrediting bodies and the Nurse Practice Act
Must comply with OSHA standards
