PDN 1:1 Licensed Practical Nurse (1st and 3rd shift available)

Job Summary
Provide 1:1 in-home skilled nursing care per the provider's plan of care, administering medications, wound care, and specialized procedures as ordered. Assess, monitor, and document patient vitals, blood glucose, and respiratory status while responding promptly to signs of infection or distress. Partner with clients, families, and the care team to support goals of care, educate on routines, and coordinate supplies and ongoing care planning. Complete accurate clinical notes and communicate shift handoff information to ensure continuity. Consistent 1:1 private duty shifts with flexible scheduling and no overtime required. Required Qualifications
- Graduate of an accredited practical (vocational) nursing program
- Current LPN/LVN license in the state(s) in which you practice
- Current CPR/AED/BLS/First Aid certification
- Able to visually and aurally observe and assess the patient
- Able to provide proof of valid driver's license and auto liability insurance if assignments include driving own vehicle to transport patient
- Able to frequently lift, reposition, and transfer patients
- Medium physical activity may require occasional lifting, pushing, or pulling up to 50 lbs
Desired Qualifications
- One year of clinical experience
- Home care/private duty experience
- Trach and/or ventilator experience
