PDN 1:1 3rd Shift Licensed Practical Nurse

Job Summary
Provide 1:1 in-home skilled nursing care per the provider's plan of care, including administering medications, performing wound care, and monitoring vital signs. Assess, monitor, and document patient conditions such as blood glucose and respiratory status, while responding promptly to signs of infection or distress. Partner with clients, families, and the care team to support goals of care, educate on routines, and coordinate supplies and ongoing care planning. Complete timely clinical notes and communicate shift handoff information to ensure continuity. Coordinate with the supervising RN and office team to support reliable coverage and excellent outcomes.
Required Qualifications
- Graduate of an accredited practical (vocational) nursing program", "Current LPN/LVN license in the state(s) in which you practice", "One year of clinical experience", "Completion of the Office's preceptor program", "Current CPR/AED/BLS/First Aid certification", "Reliable transportation to/from care sites and/or work locations", "Valid driver's license", "Auto liability insurance
