Job Summary
As a PD-Bridge Services Specialist, provide a safe environment for residents and assist in daily living and rehabilitation services. Responsibilities include conducting client assessments, facilitating therapeutic programs, coordinating care with various agencies, and ensuring compliance with health regulations. Perform administrative duties such as maintaining clinical documentation, engaging clients and families in treatment, and managing crises. Candidates should have experience working with individuals with mental illness and provide exemplary customer service while being able to work independently and collaboratively with a team.
Required Qualifications
- High School diploma, GED, or equivalent education plus five years’ experience; OR Associate’s degree plus three years’ experience; OR Bachelor’s degree plus two years’ experience; OR Master’s degree in a related field plus one year’s experience, working in a human service setting.
- Eligibility for MHRS certificate.
- First Aid Certification or successful completion of First Aid Certification within 30 days of hire.
- Meet government and/or regulatory standards for continuing education credits and recertification, including a minimum of 20 hours of directly related professional training, if assigned to a crisis residential facility.
Additional Requirements
- Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Momentum practices.
