Job Summary
Deliver comprehensive outpatient care to adult and senior patients in a team-based, value-driven primary care setting. Maintain accurate medical records, diagnose and manage moderately to complex medical conditions, coordinate referrals with specialists, participate in clinical quality improvement initiatives, and collaborate with interdisciplinary teams to ensure holistic, patient-centered care. Exercise independent clinical judgment, contribute to strategic care delivery initiatives, and support a culture focused on improving patient experience and outcomes. Full-time, outpatient, Monday–Friday, 8:00 AM–5:00 PM, with competitive compensation including sign-on bonus, relocation assistance, and comprehensive benefits.
Required Qualifications
- MD or DO from an accredited medical school
- Active, unrestricted medical license in the state of practice
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine
- Minimum of 2 years' experience in value-based care or managing high-acuity geriatric patients
